Managing of paediatric Alopecia Areata (Khalitya) through Ayurveda: A case report

Digvijay Singh, Kavya Mohan.

Abstract
Background: Khalitya (alopecia areata), is a prevalent condition that affects individuals of all ages and significantly impacts quality of life. Modern management of hair loss often relies on steroid-based or surgical treatments, which may yield temporary relief with potential adverse effects. Objective: This case report evaluates the efficacy of Ayurveda management protocol in paediatric alopecia areata. Case details: A 5-year-old male presented with patchy hair loss (Alopecia areata) of one-month duration, unresponsive to 5% minoxidil topical cream prior. On Ayurveda evaluation, the condition was diagnosed as Pittaja Khalitya (type of alopecia caused due to Pitta body humor). The patient underwent a 45-day treatment protocol involving oral administration of Rasayana Vati and Saptamrit Lauha, external application of Bhringraj Taila, Triphala and Palandu Kharshana (rubbing), along with Nasya (nasal instillation) with Yashtimadhu Taila, Matrabasti (medicated oil enema) with Keshya Taila, Shirodhara (pouring of medicated oil on the forehead) with Bhringraj Taila, and a single session of Jalaukavacharana (leech therapy). Outcomes: Clinical assessments based on Keshabhumi (scalp condition), grading scales and the modified Norwood-Hamilton Scale demonstrated marked improvement in hair regrowth, scalp condition, and reduction in hair fall. No adverse events were reported. Parent feedback indicated high satisfaction with the treatment outcome. Conclusion: The results underscore the efficacy of a multimodal Ayurveda regimen targeting the root causes of Khalitya through Dosha-specific interventions. Herbomineral Rasayana provided systemic support by nourishing Asthi Dhatu, while Panchakarma therapies aided in Dosha elimination, microcirculation enhancement, and follicular rejuvenation. There was no adverse reaction observed during the course of treatment. Further controlled clinical trials are warranted to validate these findings on a larger scale.

Key words: Ayurveda, Alopecia areata, Rasayana, Panchakarma, Nasya, Basti, Leech Therapy, Scalp Health.
