Therapeutic efficacy of two polyherbal Unani formulation in the treatment of Tinea corporis infection: A randomised, single-blind, comparative clinical trial

Abstract
Background: Tinea corporis is a highly prevalent superficial fungal infection of the skin that poses a considerable burden on society. Although effective, modern therapeutic options are increasingly associated with the development of drug resistance and various adverse effects such as hepatotoxicity and phototoxicity. In this context, Unani therapy may serve as a safe and effective alternative. Objective: To evaluate and compare the efficacy of two polyherbal Unani Zemads (PUZ-1) and PUZ-2 with Joshanda Haleela Siyah (JHS) in the management of T. corporis. Methods: A randomised, single-blind, comparative clinical trial was conducted among 40 patients aged 18-60 years diagnosed with T. corporis infection. Participants were divided into two groups. Group A participants received JHS (15 g once daily) along with PUZ-1 (once daily local application for 10 min). Group B received the same oral formulation with topical PUZ-2. The intervention was administered for 3 weeks, with weekly follow-up assessments. Treatment efficacy was evaluated using the global assessment scale, KOH examination, and photographs of the lesion. The trial was approved by the Institutional Ethical Committee (NIUM/IEC/2021-22/031/JT/06,) and registered in Clinical Trial Registry of India (CTRI/2023/06/053844). Results: Both groups demonstrated a statistically significant reduction from the baseline in all outcome measures (p < 0.001). Although the intergroup differences were not statistically significant (p>0.05), group A showed comparatively better clinical improvement in terms of pruritus reduction. Conclusion: Both PUZ ointments and JHS were effective in the management of T. corporis, resulting in a significant reduction in all the outcome measures. However, JHS with PUZ-1 demonstrated a relatively faster response to pruritus reduction.
