Abstract
Background: Decompensated liver cirrhosis represents an advanced stage of chronic liver disease characterised by portal hypertension, ascites, and impaired hepatic function. Conventional management primarily focuses on symptomatic relief through diuretics and paracentesis, with limited reversal of disease progression. In Ayurveda, this condition can be correlated with Yakritodara (hepatomegaly) progressing to Jalodara, (ascites) involving derangement of Agni (metabolism) and obstruction of Srotas. (body channels) Objective: to evaluate the clinical outcomes of an Ayurveda interventions in refractory decompensated cirrhosis using biochemical, radiological, and Child–Pugh prognostic parameters. Case Details: A 58-year-old male with a history of Type 2 Diabetes Mellitus and Hypertension presented with abdominal distension, scleral icterus, and Grade 3 bilateral pedal oedema. Despite standard allopathic treatment, symptoms persisted. The patient was managed with a four-week Ayurveda regimen which included Nitya Virechana (daily purgation with Trivrutastak Churna), Patoladi Kwatha, Mahatikta Ghrita, and Indrayavadi Vati internally, along with dietary regulation. Results: Clinical assessment demonstrated reduction in abdominal girth and pedal oedema. Biochemical parameters showed improvement, with total bilirubin decreasing from 4.96 mg/dL to 2.58 mg/dL and normalisation of transaminases. Ultrasonography revealed reduction in ascitic fluid from moderate–gross to mild. Child–Pugh score improved from Class C (10) to Class B (8). Conclusion: This case report demonstrates that Ayurveda management may contribute to improvement in clinical and biochemical parameters in refractory decompensated cirrhosis. Further controlled studies are required to validate these findings.
